Community Rehabilitation and Treatment

What is Community Rehabilitation and Treatment (CRT)?
The CRT program provides clinical, case management and other support services to adults coping with and recovering from the impact of severe and persistent psychiatric conditions.

The Goal of CRT:
The mission of the CRT program is to help people with persistent psychiatric difficulties to live fully in the Addison County community. We do this by providing flexible and respectful services based upon individual values, goals, and strengths.

Description of Services:


Community Support Services
These services include individualized planning, coordination, and community based support services as determined by the consumer/client’s identified goals.

Psychiatric services
CSAC psychiatrists and psychiatric nurse practitioners provide assessment and medication management of major psychiatric symptoms and conditions.

Individual and Group Counseling
Individual and group counseling is available to work on life goals and challenges, strategies for coping with symptoms and stressors as well as other issues identified. Specialized areas of focus include Dialectical Behavioral Therapy (DBT) and individual and group counseling for co-occurring mental health and substance abuse challenges.

Vocational Services
Addison County Employment Services (ACES) assists in individualized job searches and job applications, and provides ongoing support for maintaining competitive employment.

Recovery and Wellness Services at Evergreen House
Evergreen House provides a welcoming setting where individuals can participate in groups and activities in support of wellness and recovery.

Residential Consultation and Support
Consultation and assistance is available regarding identifying available housing and housing related resources and supports. The CRT Program staffs and manages two group residences, Hill House a transitional group residence, and Robinson House which is a licensed “Therapeutic Community Residence”.
