The Adult Stabilization Program (ASP) is a short-term, targeted case management program, which provides community and office based support to Addison County residents. ASP is a strengths-based and solution-focused program that supports clients in solving problems and developing realistic, achievable goals.
The program works closely with CSAC’s Emergency Team and Intake Coordinator to provide at-risk clients in need of immediate assistance mental health evaluations and access to clinical support.
Role of ASP case manager
- Provide stabilization services such as case management, system navigation, advocacy, and supportive counseling to voluntary adult clients in community and/or office based settings.
- Engage clients in goal-oriented activities to increase overall levels of functioning and self-sufficiency.
- Collaborate with service providers and natural supports (such as family and friends) to deliver intensive, short-term case management for individuals encountering multiple treatment barriers or presenting with complex needs.
